Abstract A 41-year-old man developed fever and vomiting (Day 1), followed by back pain and left-sided weakness on Day 2. On Day 15, the patient's left hemiparesis deteriorated. Magnetic resonance imaging (MRI) of the brain revealed right cerebral infarction and multiple vascular stenoses in both hemispheres. Spinal MRI showed spinal subdural hematoma (SSDH). The patient was diagnosed with reversible cerebral vasospasm syndrome (RCVS). Localized convexity subarachnoid hemorrhage occurred on Day 22. The left hemiparesis and multiple cerebrovascular stenoses completely resolved. He discharged on Day 87. Cerebral vasospasms related to spinal hemorrhage were discussed, and reappraisal for this condition would be needed.
