three cases presenting with EIMFS; first case was treated with potassium bromide and two of whom were treated with potassium bromide+ketogenic diet (KD). Case 1 was a 2-month-old girl who was presented with intractable partial seizures. Despite midazolam infusion, phenytoin and levetiracetam seizures like eye blinking, clonic jerks in arms, nystagmus, orobuccal movements continued. Phenobarbital, pyridoxine, clonazepam, topiramate, clobazam,folinic acid and ACTH were the other given antiepileptic drugs which were uneffective. Potassium bromide was started in a dose of 40mg/kg and then increased to 80mg/kg dose. Potassium bromide showed to be effective in 3 weeks causing marked reduction in seizure frequency. Case 2 was a 45day-old girl who was suffered from multiple seizures like eye blinking, oromotor movements, limb clonias. Phenobarbital, phenytoin, levetiracetam, clonazepam, clobazam, topiramate, pyridoxine and midazolam infusion didn't show any benefit. Potassium bromide was started in a dose of 10mg/kg and then increased to 30mg/kg dose. Because of side effects we had to slow down the increasing rate. KD (4:1 ratio) was added 3 weeks later. Combination of potassium bromide(50mg/kg) with KD were satisfactorily responsive. Case 3 was a 66-day-old girl who experienced migrating clonic partial seizures. Seizures were refroactory to casual antiepileptics. Potassium bromide was started in a dose of 30mg/kg and then increased to 80mg/kg. Even though seizure frequency reduced slightly, there was no dramatic response. KD (4:1 ratio) was started. Combination of potassium bromide and KD showed benefit. Management of these intractable seizures in EIMFS patients may be difficult. Alternative treatment choices should be kept in mind.
