Experts’ summary: The authors conducted a multicenter randomized controlled trial comparing cystocele repair with transvaginal synthetic non-absorbable mesh versus laparoscopic sacral colposuspension [1]. Surgical complications were chosen as the primary outcome, evaluated using a modified Clavien-Dindo classification [2], after 12 mo of follow-up. Secondary outcomes were the reintervention rate and functional and anatomic results. A total of 257 women were included in the study and randomized to two groups. Fewer Clavien grade 2 complications were observed after laparoscopic procedures, although the difference was not statistically significant (17% vs 26%). Grade 3 or higher complications were equally rare in both groups. Functional outcomes (symptoms, quality of life, and dyspareunia) did not differ by group. Laparascopic sacrocolpopexy provided better apical support compared to transvaginal mesh surgery.
