Highlights • Gallstone ileus is seen in the elderly and has a high mortality rate.• Radiographic features of gallstones are variable and can mimic other pathologies.• A high index of suspicion for gallstone ileus in the elderly should be maintained despite contradictory surgical history.• Surgical exploration in the setting of small bowel obstruction is paramount to the management of gallstone ileus.
