Highlights • Priapism is a condition characterized by ≥4 hr continues penile erection, without sexual stimulation.• The most common causes of ischemic priapism is the idiopathic.• The presence of extra-urological signs and symptoms must suggest malignancy.• The main step is establishing specific treatment of the underlying disease.• Surgery is reserved only if conservative therapy was unsatisfactory.
