Highlights • Sclerosing mesenteritis (SM) is difficult to be diagnosed preoperatively if careful attention is not given at imaging ordered.• Vessel encasement seen on imaging is not definite till you confirm it during surgery.• Debilitating disease even with complete excision.• Long term follow up is required with good palliative care.
