Highlights • High dose barbiturate use has been linked to life threatening hypokalemia and rebound hyperkalemia.• Pentobarbital is a barbiturates that is used in high doses to induce a coma state to lower intracranial pressures.• Through constant monitoring of potassium levels and permissive hypokalemia, avoidance of rebound hyperkalemia is achievable.
