Highlights • There is little information regarding severe intraventricular hemorrhage casting associated with moyamoya disease.• Performing only external ventricular drainage in such a case is preferred way, however, there are no scientific date supporting this rule.• This is the description of aspiration of severe intraventricular hemorrhage in moyamoya disease patients in acute phase.• Our two cases suggested that early endoscopic aspiration of severe intraventricular hemorrhage may prevent poor outcomes.
