Highlights • Our patient clinically presented with stage 2 pyomyositis.• In immunocompromised patients, detection of early stage pyomyositis is challenging.• Most patients present at stage 2 or 3, which can increase risk of complications.• CT scan an acceptable radiological modality to detect presence of pyomyositis.• Surgical intervention, broad-spectrum antibiotics suggested for stage 2 pyomyositis.
