Highlights • Intramuscular hydatidosis is an extremely rare manifestation of hydatid disease which is endemic in India.• Patient presented with a large swelling over the left thigh and was diagnosed in a timely manner using a multimodal approach.• The rare location of the disease is confounding and required thorough and meticulous evaluation.• Upon surgical exploration, the presence of multiple cysts and daughter cysts proved to be challenging but complete pericystectomy was achieved.
