Highlights • Peliosis lienalis is a rare pathological entity which may be associated with chronic myelomonocytic leukemia.• Increasing splenomegaly in patients with hematological malignancies should rise suspicion of impending splenic rupture.• Peliosis lienalis may be suspected with new inhomogeneous splenic parenchyma on sonography.
