Highlights • A kidney transplant recipient presented with abdominal pain and vomiting.• Computed tomography confirmed a diagnosis of renal paratransplant hernia.• Incarcerated small bowel was seen in the retroperitoneum via a peritoneal defect.• Immunosuppressed patients can benefit from the laparoscopic treatment of RPH.
