Treatment efficacy for AA of the moustache was described in 1 article with a very low level of evidence in 2 patients, with treatment resulting in only partial regrowth of the treated areas (Table II). Overall, most studies had a small number of cases and lacked control groups and randomization. Follow-up to establish treatment response and disease remission was unavailable for most studies. All patients treated with DPCP and anthralin had reported tolerable localized pruritus, vesicles, bullae, or a combination of these during therapy. Some of these patients experienced widespread dermatitis (29.8%), widespread pruritus (31.9%), regional lymphadenopathy (51%), hypopigmentation (38.3%), and flu-like symptoms (10.6%). One patient (5.6%) with beard AA who received intralesional triamcinolone developed skin atrophy, 1 patient (6.7%) taking systemic corticosteroids developed hypertension, 1 patient (6.7%) developed myalgia, and another patient developed (6.7%) facial swelling. Transient hyperemia and eye irritation were reported in some patients who received prostaglandin analogue. Patients who were treated with photodynamic therapy and laser surgery did not experience any major adverse effects. In conclusion, treatment with DPCP combined with anthralin demonstrated the best overall result for the level of evidence and treatment response for AA of the eyelashes, eyebrows, and the beard. In addition, patients were more likely to experience a longer remission and earlier regrowth in treated areas than after treatment with DPCP alone. It was unclear how patients with AA of the eyelashes achieved regrowth without direct application of this treatment to the eyelids. Given the potential adverse effects of DPCP and anthralin, good education and supervision are necessary to ensure the safe use of this treatment.
