OBJECTIVES This study aimed to characterize insertional mutations disturbing mgrB in carbapenem-resistant K. pneumoniae (CRKp). METHODS One hundred eighteen clinical CRKp strains were surveyed for polymyxin resistance and insertion sequences (IS) disrupting mgrB. RESULTS Seventy-eight (66%) strains displayed polymyxin resistance, of which 53% (42/78) had IS::mgrB inserts. Sequencing analyses showed 13 insertion sites in mgrB. For the first time, ISsen4(IS3)::mgrB was observed in CRKp. CONCLUSIONS Ten different ISs disrupted mgrB with predominance (76%) of IS5 sequences.
