n this commentary, I will pose two sets of questions. The Ifirst set is after the fetus is delivered but before placental separation. What procedures or tests should we be doing, and when and how dowe obtain blood for cord blood gases and/ or lactate? When do we clamp the cord, and should we do umbilical cord blood banking or umbilical cord tissue collection? The second set of questions is related to the care of the neonate. What is the new evidence or protocol for neonatal assessment care? What is the clinical newborn assessment or care priority? How do we assess fetal/neonatal academia, placental transfusion and neuro-protection, and cordblood collection for possible future stemcell transfusion?
