To the Editor: We commend Ravindran et al. for providing a novel insight on the connection between monoclonal gammapathy and thrombotic microangiopathy (TMA). How monoclonal Ig may facilitate the occurrence of TMA remains debatable. Next to endothelial injury, inhibitor activity such as in thrombotic thrombocytopenic purpura or interference with proteins involved in coagulation, dysregulation of alternative complement pathways may also represent a plausible mechanism linking Ig and TMA. Yet, in the opinion of the authors, complement dysregulation appears to be indicative of Ig-related C3 glomerulopathy rather than Ig-associated TMA. Accordingly, only one
