Introduction The intracranial tuberculomas represent one of the most serious localizations of tuberculosis, their incidence varies according to the context accounting for 0.2% of the intracranial processes in the Western countries and 5 to 10% of the intracranial masses in the countries in the process of development. Patient and method We report the case of a 60-year-old woman, whose husband had contracted the pulmonary tuberculosis five years ago. She suffers from headache, a right hemiparesis with a brachial prevalence evolving for 03 months and two episodes of epileptic fits. The cerebral IRM showed an expansive, a right prerolandic and a very swollen process, which is in favor of a malignant glial tumor or a secondary localization. The extension's assessment is negative. The surgery act is recommended. Results The patient was successfully operated. The intraoperative aspect shows a nodular and closes yellowish lesion. The histological study concludes as a tuberculoma. Conclusion The intracranial tuberculoma is an unusual variety of tuberculosis. The positive diagnosis is difficult without evocative clinical context and specific radiological image. It is based on a beam of presumption clinicobiological and radiological arguments. The confirmation remains histological. The prognosis is related to the rapidity of the diagnosis, the quality of the surgical ablation and the antibacillary treatment.
