A 25-year-old man was admitted to the emergency department with impaired consciousness, right hemiplegia and sudden loss of speech preceded by a moderate headache that began after he smoked about 20 cannabis joints. History taking revealed irregular alcohol intake and tobacco smoking during the last 4 years and cannabis use during the last 5 years. The patient reported that he occasionally smoked 6 to 10 joints. The emergency admission had occurred approximately 10 hours after symptom onset. The physical examination at admission found the following: Glasgow score 9/15; body temperature 37.5 8C; blood pressure 110/80 mmHg; upper right limb motor deficit with a proportional right upper motor neuron syndrome that also affected sensitivity during the flaccid phase; NIHSS score = 10. The emergency brain computed tomography visualized an intraparenchymal hematoma in the left ventricular nucleus and an image in the right frontal cortex (Fig. 1). Brain magnetic resonance imaging showed the left lenticular hematoma, a right paraventricular hematoma, and several point hemorrhagic foci in the right frontal cortex
