Abstract Primum non nocere, or first do no harm, is a core principle for all of us who provide healthcare to patients. But what do we say and do when a medical error has caused harm to a patient. Over the last 10 years, "Disclosure" of medical error to the patient and family has become an important practice for the physician and a foundational component of the Quality and Safety mission in medicine. There is a skill in leading transparent disclosure conversations so that they are constructive and valuable to the patient, family, physician and healthcare team. These concepts, as well as the multidisciplinary process and procedure of disclosure, are addressed in this article. It is never easy to talk with patients and family about how our care may have caused harm, but it is the right thing to do.
