A 61 year-old female kidney transplant (KT) recipient developed radiologic and biopsy-confirmed cortical necrosis in her transplant with elevated serum creatinine during concurrent chemo-radiotherapy (CRT) for anal squamous cell carcinoma (SCC). Acute anatomical and pathological changes within a transplanted kidney during CRT have not been described in the literature previously. Further research is required to ascertain the potential significance and long-term clinical consequences of such findings.
