Highlights • The opt-out policy was difficult to implement and not well received by NHS staff.• The purpose of the policy was poorly understood, and NHS staff did not know how data on decliners was being used.• NHS staff were concerned that the the policy could be used to ‘name and shame’ decliners.• Redeployment of unvaccinated NHS staff was found impractical due to staffing levels.• Workforce engagement in designing flu vaccination approaches is key to their success.
