The purpose of this paper is to report on a patient treated with hypothermic cardiac standstill for clipping of a ruptured, giant, previously coiled basilar aneurysm. It is concluded that the excellent clinical and angiographic outcome maintained at 15 years after the operation justifies its selective use.
