Hydroxychloroquine (HCQ), a common anti-rheumatic medication, can be cardiotoxic and cause systolic or diastolic heart failure (HF). A 65 year old man presented with acute decompensated HF. He had a history of rheumatoid arthritis, and had been treated with hydroxychloroquine for the last 8 years.
