We present a case of incidental large left circumflex artery (LCx) aneurysm and left ventricular-coronary sinus fistula in a previously healthy asymptomatic 53-year-old adult who had a routine CT of the abdomen as part of the work-up for hematuria. Coronary CT angiography revealed a large aneurysm measuring 3.8 × 3 cm in maximal axial dimension arising from the mid-LCx and situated adjacent to the left atrium in the left inferior pulmonary vein and the left atrioventricular groove (figures 1a-d). The walls of the aneurysm were partially calcified and there was some layering noncalcified plaque along the walls the aneurysm. The LCx did not exhibit obstructive disease.
