Ventricular fibrillation (VF) caused by Prinzmetal angina is a rare cause of sudden cardiac death in patients without apparent structural heart disease but should be excluded. Despite long-term medical treatment, coronary spasm-induced ventricular arrhythmias may reoccur. This justifies an ICD implantation. However, in the acute setting, only the treatment of the coronary spasm will prevent new clusters of life-threatening arrhythmias.
