We describe the case of a diabetic 63-year-old man who underwent routine transthoracic echocardiography. He was asymptomatic. Physical examination was normal. The electrocardiogram showed sinus rhythm with few supraventricular extrasystoles. The transthoracic echocardiogram revealed an isolated enlargement of the right atrium (RA; Figure 1; panel A). Left systolic function was preserved without dilation of the left cavities nor left or right valve disease. Right ventricular function and size were normal
