Abstract Intravascular large B-cell lymphoma is a rare disease that often goes unnoticed and is repeatedly diagnosed during autopsy. Patients present with a wide variety of nonspecific symptoms, which makes the diagnosis challenging. This case presents a patient who was initially diagnosed with fever of unknown origin but later developed a retiform purpuric rash that prompted the diagnosis.
