Abstract Introduction: Laryngeal clefts (LC) are upper respiratory malformations predominately found in the posterior laryngeal wall. The frequency is 1:10,000, more frequently affect males, and can be syndromic features. There is no report of a transfixing anterior laryngeal cleft. Case report: This diabetic mother at full-term pregnancy delivered a stillborn macrosomic 4780 g dysmorphic stillborn male with left renal agenesis, aortic coarctation, and anterior laryngeal cleft. Conclusion: Anterior laryngeal clefts can occur, and in this case, occurred in association with renal agenesis and maternal diabetes.
