The features of diphenhydramine poisoning are well described [1]. Although the elimination half-life of diphenhydramine has been calculated after therapeutic dose [2–6], it has not been estimated in overdose. We report the clinical course, diphenhydramine concentrations and elimination half-lives in two patients after diphenhydramine overdose. Toxicological screening was performed using ultra-performance liquid chromatography (Waters ACQUITY; Waters, Co., Milford, MA) with tandem mass spectrometry (Waters TQD detector) and exact mass time of flight (Waters Xevo G2 QTof) and LC-MS/MS was employed to quantify drugs present.
