ABSTRACT Introduction: Endophthalmitis is a rare but an extremely serious complication of intraocular surgery. Despite continuous improvements in its management visual outcomes remain poor. Clinical trials of prophylactic measures are difficult to conduct due to low incidence and the large patient numbers are required to elucidate significance. There is limited high order evidence regarding most effective prophylaxis of postoperative endophthalmitis and there is a significant variation in practice. Every surgeon constantly aims to prevent and eliminate postoperative endophthalmitis based on available guidelines and local protocols. Areas covered: In this article we have reviewed the published literature covering trials, reports, international guidelines, surveys of national and international practices and recommendations from national associations and colleges. Expert commentary: We have summarized, in a single flowchart, a pragmatic list of important prophylactic preoperative, intraoperative and postoperative steps for preventing this devastating complication and provide effective and safe prophylaxis.
