A 21-year-old Taiwanese woman presented with a rapidly growing, firm, erythematous neck mass that had persisted for six years. Surgical excision with clear margins was performed, and histopathological examination confirmed pilomatricoma. This case underscores the diagnostic challenges of pilomatricoma in atypical presentations and highlights the importance of clinical awareness for accurate diagnosis and appropriate management.
