A 27-year-old male presented with a large necrotic ulcer in the perineal area, scrotum and, buttocks for the past two and a half months. The ulcer was associated with severe pain, making it difficult for him to walk or lie down. He also complained of purulent discharge from the lesion, which used to soak his clothes.
