The 2020 ESC Guidelines and the 4th version of the Universal Definition of Myocardial Infarction (UDMI) promote high sensitivity cardiac troponin (hs-cTn) assays over conventional sensitivity cTn assays. Obviously, the higher analytical sensitivity facilitates the detection of smaller infarcts and earlier diagnosis of MI compared with less sensitive assay generations. Even after uncomplicated elective cardiac surgery, cTn is rapidly released into the blood in high concentrations, reaching peak values within 12–24 h, depending on the Type 5 MI (at least 1 ancillary criterion required) Typical release curve of hs-cTn a er on-pump CABG (wash-out and myocardial injury of variable extent)
