We report an atypical case of recrudescence of Plasmodium falciparum in a migrant with 4% parasitemia, 7 years after leaving endemic country. This case supports the hypothesis of recrudescence of a latent and persistent sub-microscopic parasitemia, triggered by time-related decreasing immunity and immunosuppression (tocilizumab).
