Learning Objectives: Acute respiratory distress syndrome (ARDS) is a life-threatening form of respiratory failure with high mortality. Because of pathological oxygen supply dependence, increasing the oxygen delivery (DO2) for ARDS patients is important. Since appropriate positive end-expiratory pressure (PEEP) may increase oxygenation and cardiac output to affect DO2. It is significant to select a appropriate PEEP in ARDS patients. Electrical Impedance Tomography (EIT) has emerged to assess the pulmonary gas distribution, which recently gained increasing interest for PEEP titrating. EIT has merit of noninvasive, radiationless and bedside aviable. It may become a promising method for PEEP titration. Methods: An incremental PEEP trial was performed to select the appropriate PEEP level in 15 ARDS patients from January 2017 to January 2018.The respiratory mechanics, hemodynamic, gas exchange, gas distribution and oxygen metabolism parameter were recorded after each Peep was selected according to ARDSnet table and minimum GI index as the ARDSnet method and EIT method. Then compare parameters of ARDSnet method and EIT method. Results: A significantly lower PEEP, higher cardiac index and DO2 were recorded in EIT method(P<0.05). While no significant difference in oxygenation between two methods(P>0.05). Conclusions: Compared with the ARDSnet method, the EIT method can improve DO2 and may become a promising methods to titrate PEEP for the ventilated patients with ARDS.
