Objectives: To study the mechanism of blood pressure variability in white coat hypertension. In order to investigate the relationship between blood pressure variability (BPV) with plasma renin activity (PRA) or aldosterone (ALDO) among the white coat hypertension (WCH), normal blood pressure (NTG) and essential hypertension (EHT) groups, we have measured the plasma renin activity and monitored the ambulatory blood pressure. Methods: 150 patients (an average age of 39.60 ± 4.74) with white coat hypertension (WCH), 161 patients (an average age of 40.38 ± 6.54) with essential hypertension (EHT) and 167 healthy people (an average age of 41.37 ± 4.70) with normal blood pressure (NTG) were selected in the Henan Provincial People's Hospital from January 2013 to December 2016.The all-day systolic blood pressure average value (ADSBPV), the all-day diastolic blood pressure average value (ADDBPV), the daytime systolic blood pressure average value (DSBPV) and the daytime diastolic blood pressure average value (DDBPV), the night systolic blood pressure average value (NSBPV) and the night diastolic blood pressure average value (NDBPV), the all-day systolic blood pressure standard deviation (ADSSD) and the all-day diastolic blood pressure standard deviation (ADDSD) are monitored, while the PRA and ALDO were determined. Lastly, analyze the correlation between ADSSD, ADDSD and PRA, ALDO among three groups using Pearson correlation analysis. Results: The results show that ADSSD and ADDSD are related to the PRA, and ADDSD is correlated with ALDO, but there is no correlation between ADSSD and ALDO, in the WCH group. Additionally, ADSSD and ADDSD are not related to PRA and ALDO in the EHT and NTG groups. Conclusion: The blood pressure variability of patients in white coat hypertension is associated with plasma renin activity.
