Objectives: Frailty is a common geriatric syndrome characterized by increased risk of cardiovascular risk factors in hypertension elderly people. However there have been few studies regarding the association between frailty and ambulatory blood pressure (BP). So, we assessed the relationship of frailty with ambulatory BP in elderly hypertension Adults. Methods: We analyzed data of 348 elderly hypertension adults(age >  = 65years)from The Geriatric Ward of our Hospital in Guangzhou. BP was determined with validated devices under standardized conditions during 24 hours. Frailty was defined as having 3 or more of the following criteria: weight loss, low grip strength, low energy, slow gait speed, and low physical activity. Associations with systolic BP (SBP),BPV, dipping(nocturnal SBP decline) and MBPS. Results: The Adults’ mean age was(84.5 ± 7.48) years (90.2% men); 37.4% were frail.Compared with non-frail adults, those with frailty had 4.1 mm Hg higher nighttime SBP,5.18% less SBP dipping (P < 0.001) and higher the anti-spoon-shaped blood type (P < 0.001).24h-SBP, daytime SBP,BPV and MBPS was not significant(p > 0.05). Conclusion: In Geriatric Ward older adults with hypertension, frailty were independently associated with higher nocturnal SBP and blunted nocturnal decline of SBP(the anti-spoon-shaped blood type). These findings may help us pay more attention to higher nocturnal SBP in elderly hypertension Adults with frailty. And explain why some study explode higher mortality associated with frail older subjects.
