available at http://www.ncbi.nlm.nih.gov/pubmed/31169141 Editorial Comment: This study evaluated whether adding a vacuum erection device (VED) to regular use of tadalafil could achieve better penile rehabilitation following posterior urethroplasty for pelvic fracture related urethral injury. Altogether 78 such patients with postoperative erectile dysfunction were enrolled and divided into 2 treatment groups. Group 1 (36 patients) used a VED combined with tadalafil and group 2 (42) used tadalafil only. The results showed that the addition of VED to regular use of tadalafil preserved more penile length statistically (mean SD 0.4 0.9 cm vs -0.8 0.7 cm, p <0.01). The 5-item International Index of Erectile Function and Quality of Erection Questionnaire scores were higher in group 1 vs group 2 (both p <0.05). After treatment 21 patients in group 1 (58.3%) and 19 in group 2 (45.2%) could complete vaginal penetration. Thus, in patients with erectile dysfunction after posterior urethroplasty the combination of VED and tadalafil seems to improve erection and increase penile length. Allen D.
