66 l Nursing2019 l Volume 49, Number 4 www.Nursing2019.com S KIN TEARS have historically been unrecognized by patients and caregivers in most circumstances, but they can become chronic and difficult to manage when left untreated.1,2 As the skin becomes drier, less elastic, and more vulnerable as one ages, skin tears become more common, particularly in older adults.1,2 This article reviews the prevalence, prevention, assessment, and management of skin tears with the goal of reducing misdiagnosis and promoting evidence-based management.
