Acute necrotizing encephalopathy of childhood (ANEC) is a rare but devastating non inflammatory encephalopathy which occurs abruptly in previously healthy individuals. In some cases death occurs within days of presentation3. Its unique clinic-radiological changes is characterized by multifocal symmetrical brain lesions including thalamus, cerebellum and brainstem tegmentum1, 2. The underlying etiology is still unclear but generally postulated as parainfectious manifestation in association with viral illness especially influenza virus3. Familial cases with chromosomal missense mutation of RANBP2 (RAN-binding protein 2) which is termed as ANE1 has been reported 4.
