In Reply to Sivanand and Saravanabavan: We appreciate Sivanand and Saravanabavan’s thoughtful comments on our letter. To clarify, we did not focus on longitudinal integrated clerkships (LICs) but, rather, on longitudinal primary care clerkships (LPCCs), during which patients presenting with acute on chronic destabilization in the outpatient setting may be underrepresented compared with patients presenting with stable chronic disease, potentially underexposing students to an important aspect of ambulatory medicine.
