Cardiovascular complications are common postoperative adverse events of clinical and prognostic importance. Among the various types of cardiovascular complications, myocardial infarction and acute myocardial injury are particularly reliant on biomarkers for their diagnosis. In the contemporary era, this biomarker is almost always either cardiac troponin T or cardiac troponin I. A nuanced understanding of postoperative troponin testing is therefore highly relevant to anesthesiologists and other perioperative physicians, especially given the absence of consensus even among experts. This review will discuss the fundamentals of troponin testing, underlying evidence for its role in perioperative care (including areas of certainty and uncertainty), and approaches for its implementation and interpretation.
