Second, during the study period, the emergency access to HTx provided to ECMO patients was limited to 4 days. This unique and nonextendable period was activated only in stabilized patients considered to be suitable HTx candidates. Conversely, current French and most international allocation schemes ensure a much longer priority period, whereas prolonged ECMO support has been associated with poorer posttransplant outcomes (7). We believe that an individualized strategy should be set up as early as possible in the management of the patient to avoid prolonged ECMO support. Our data provide the first elements to demonstrate that a direct transplantation strategy may be a valuable therapeutic option for ECMO patients.
