e632 www.ccmjournal.org July 2020 • Volume 48 • Number 7 preand posttransplant ECMO support. Second, during the study period, the emergency access to HTx provided to ECMO patients was limited to 4 days. This unique and nonextendable period was activated only in stabilized patients considered to be suitable HTx candidates. Conversely, current French and most international allocation schemes ensure a much longer priority period, whereas prolonged ECMO support has been associated with poorer posttransplant outcomes (7). We believe that an individualized strategy should be set up as early as possible in the management of the patient to avoid prolonged ECMO support. Our data provide the first elements to demonstrate that a direct transplantation strategy may be a valuable therapeutic option for ECMO patients. However, the context of our study (2) should be taken into account when analyzing our results: read the fine print before signing! Dr. Leprince received funding from Medtronic and SynCardia. The remaining authors have disclosed that they do not have any potential conflicts of interest.
