Divided nevus, also called kissing nevus, is a rare congenital and melanocytic form that occurs on adjacent parts of the upper and lower eyelid. It can have a dramatic psychological and physical impact such as ectropion, ptosis, or epiphora. Furthermore, the divided nevus has a potential risk of malignant transformation. Therefore, surgical excision is recommended for divided nevus. However, reconstruction after excision remains a technical challenge. We hereby report an innovative approach for the treatment of a large divided nevus that was successfully excised and reconstructed in a single-stage approach. A 24-year-old woman presented with a divided nevus of her left eye, involving two-thirds of the upper eyelid, upper eyebrow, and the medial part of the lower eyelid. The divided nevus also extended vertically to the tarsal conjunctiva from the upper and lower lids (1 and 2 mm, respectively) (Figure 1A). With closed eyes, the diameter of the lesionwas 3.5 ยท 2.0 cm. The nevuswas darkly pigmented and mildly thickened, and the eyebrows were slightly asymmetric. The patient reported of no ophthalmic symptoms.
