In this article, the authors report their successful surgical treatment of 2 patients with extensive and recalcitrant dissecting cellulitis of the scalp. In both patients, the authors performed staged surgical excisions under local anesthesia. Initially, the problem sinus tracts were outlined and then excised to the level of the periosteum. Pulley sutures were used for approximation and hemostasis, followed by the placement of dermal sutures using polydioxanone or polyglactin 910. One of the patients underwent 4 excisions and the other underwent 3. Procedures were performed every 2 to 3months “to allow for adequate healing time and tissue redistribution on the scalp.” Both patients had a significant reduction of drainage and were very satisfied with the cosmetic result.
