A shortened, wider pedicle could have improved her distal flap perfusion and minimizing extension on to the infratip and thinning would help diminish distal flap risk as well. Doppler ultrasound of the vascular pedicle, although not used in this case, could be a useful evaluation tool in assessing patients with poor perfusion and/or comorbid factors.
