The authors’ patient is a 65-year-old Caucasian woman who received Mohs surgery for a biopsy proven nonmelanoma skin cancer of the nasal tip (Figure 1). After clear margins were obtained, the resulting defect was 5.6 cm wide by 3.6 cm tall involving the full thickness of the right ala, right nasal sidewall, and medial cheek (Figure 2). The medial cheek portion of the defect was closed using an advancement flap repair. The full-thickness defect of the ala and sidewall was closed using a composite graft from the right ear to reconstruct the nasal lining and cartilage followed by a paramedian forehead flap.
