The oculocardiac reflex (OCR) is a dysrhythmic cardiac response to physical stimulation of the nerves of the eye or periorbital tissue that most commonly results in bradycardia. Despite its mention in the ophthalmology and oral and maxillofacial surgery literature, theOCRhas not been reported in the setting of Mohs surgery and is likely an underrecognized phenomenon. We report a case in which this reflex manifested as severe bradycardia leading to asystole requiring cardiopulmonary resuscitation in a patient undergoing resection of a recurrent basal cell carcinoma of the skin overlying the orbit.
