Mohs micrographic surgery offers a superior cure rate compared with WLE; however, its application can be difficult as EMPD lesions are often multifocal and large, resulting in lengthy and incomplete excisions; therefore, several modified techniques have been developed, but none have yet been proven to improve cure rates or surgical morbidity compared with traditional MMS. We report a novel approach to optimize the management of EMPD, which combines RCM tumor mapping, followed byMMS. Extramammary Paget disease has been shown to exhibit specific diagnostic features on RCM, and delineated tumor margins seem to correlate well with histopathological margins. Terrier and colleagues used RCM to successfully map clinical margins of genital EMPD before WLE. The theoretical benefit of targeting EMPD by RCM before MMS is noninvasive estimation of tumor continuity and extent, minimizing both the number of MMS stages required and the likelihood of additional surgery.
