Transconjunctival Approach to Scleral Fixation of Posterior Chamber Intraocular Lenses Using Gore-Tex Suture When there is insufficient capsular support for placement of a posterior chamber intraocular lens (PCIOL), options for secondary intraocular lens (IOL) implantation include anterior chamber, iris fixation, and scleral fixation.1 Recently, a technique using Gore-Tex suture for scleral-fixated lenses has gained popularity because of demonstrated safety and stability.2,3 The techniques used for placement of these lenses require a conjunctival peritomy and a significant amount of intraocular suture management. We present here a modified surgical technique for scleral-fixated PCIOLs using Gore-Tex suture and a four-trocar transconjunctival method that can be performed in conjunction with standard three-port pars plana vitrectomy.
